Dear Citizens of Bradford County,

On behalf of Bradford County Emergency Medical Service, we would like to encourage you, your family and neighbors to become an active participant in our Vial of Life project.  The Vial of Life is a free and simple tool used to aid EMS with treatment in the event that you or a loved one should ever call 911.  Accidents are not planned and they can happen to anyone.  By planning ahead of time, our mission to better serve the Citizens of Bradford County grows stronger.

Please read the following information to find out how you can better protect yourself by participating in the Vial of Life.  When a 911 call is dispatched, EMS is only given the information the caller is able to provide.  Keep in mind that in many situations the caller may not be able to provide critical information about you, the patient.          

The Vial of Life is a single page, detailed form containing your medical information which is placed on the outside of your kitchen refrigerator in a specially marked bag. Emergency workers can quickly identify if you have the Vial of Life once you have registerd the form below and placed this bag on the outside of your refrigerator.     

Should I have a Vial of Life even if I have little or no medical conditions? Absolutely! Everyone in your home is encouraged to have the form filled out and kept updated.  Knowing that you have no preexisting conditions is just as important for us to know.
